Cody Allen Phillips, 21, died July 20.

Cody is survived by parents, David and Pamela Phillips of Lake City; wife, Kerrie Phillips of Lake City; daughter, Raylee Phillips; grandparents, Lonnie Phillips of Lake City, Janie Phillips of Clinton and Paul Smith of Lake City.

A celebration of life service was held July 24 at Pine Hill Baptist Church with Rev. Lynn Mowery officiating.

Hatmaker Funeral Home was in charge of the arrangements.
